PROCEDURE FOR PROVISION

To provide rehabilitation aids to the territorial branch of the Fund (SOBEZ), AH, ASC, the following documents are submitted together with the application:
(according to the resolution of the Cabinet of Ministers of Ukraine dated 05.04.2012 No. 321 as amended)

  • A passport of a citizen of Ukraine or a document replacing it (temporary certificate of a citizen of Ukraine, permanent residence permit, temporary residence permit, refugee certificate, certificate of a person in need of additional protection, birth certificate for children under 14 years of age).
  • A copy of the taxpayer registration card (identification code) (or a copy of the passport of a citizen of Ukraine for individuals who, due to their religious beliefs, refuse to accept the registration number of the taxpayer registration card and have officially notified the relevant regulatory authority about this and have a mark in their passport).
  • Conclusion on the need to provide rehabilitation aids, which is formed by one of the following entities: MSEK, VLK, LKK, multidisciplinary rehabilitation team (if there are documents issued by December 15, 2023, confirming the need to provide rehabilitation aids - the conclusion is not submitted).
  • Certificate confirming the person's right to benefits (if available).

About us

Prosthetics Center "Ortoped"

The company specializes in the manufacture of prostheses, orthoses, orthopedic shoes and rehabilitation aids for people with disabilities. Within the framework of the state program "Ortoped" provides combatants and people with disabilities with modern prostheses, orthoses, orthopedic shoes and rehabilitation aids free of charge.

The company has its own production facilities and branches almost throughout Ukraine.

In Poltava, there is a modern medical and production center with a 50-bed inpatient rehabilitation department, where patients receive comprehensive care, round-the-clock medical support, psychological support and individual classes.

Up to 1,200 pairs of orthopedic shoes, 200–250 prostheses, about 200 orthoses and 450 rehabilitation aids are manufactured monthly. Modern technologies are used for production: 3D scanning, 3D printing, CNC machining, electronic knee assemblies and myoelectric prosthetics.

The team of specialists constantly improves their qualifications in leading training centers in the world, participates in international online courses and master classes, and are members of ISPO and Human Study. The company has a license from the Ministry of Health No. 331 and ISO 13485 and ISO 9001 certificates.
