To provide rehabilitation aids, the following documents are submitted to the territorial branch of the Fund (SOBEZ), ATC, ASC together with the application:
(according to the resolution of the Cabinet of Ministers of Ukraine dated 05.04.2012 No. 321, as amended)
- Passport of a citizen of Ukraine or a document replacing it.
- Copy of the taxpayer's registration card.
- Conclusion on the need to provide rehabilitation aids.
- Certificate confirming the person's right to benefits (if any).
For military personnel additionally:
- Certificate of the circumstances of the injury.
- Military ID card.
- Certificate of registration of IDPs.
